Annual Wellness Exams

Preventive evaluations, health history review, screening guidance, risk assessment, and individualized care planning.

Unlimited Primary Care Visits

Medically appropriate in-person and telehealth visits for covered preventive, chronic, and acute primary care needs.

Extended Telehealth Access

Daily telehealth availability from 8 AM–10 PM CST for eligible non-emergent concerns and follow-up care.

Medication review, routine refill requests, treatment monitoring, and medication adjustments when clinically appropriate.

Ongoing support for hypertension, diabetes, thyroid disease, asthma, obesity, high cholesterol, and other common conditions.
