Emergency and Hospital Care
Emergency department visits, ambulance services and hospitalization are outside the primary care membership.
Specialists and Surgery
Specialist fees, surgery and procedures performed outside 24Sculpt Health are not included.
Outside Testing and Medications
Outside laboratory fees, imaging charges and prescription medication costs may require separate payment or insurance coverage.

How do I know whether I am enrolled?
Contact your employer or benefits administrator to confirm your eligibility, enrollment status and membership start date.
Do I pay a copay for an included visit?
No separate copay is charged for primary care services included in your employer-sponsored membership. Services outside the membership may involve additional charges.
Can every concern be treated through telehealth?
No. Some concerns require an in-person examination, testing, imaging, emergency care or specialist evaluation. The appropriate visit type will depend on your symptoms and clinical needs.
Can I use the membership for medication refills?
Routine refill requests may be addressed when clinically appropriate. An evaluation, monitoring or laboratory testing may be required before certain medications can be prescribed or renewed.
Are laboratory tests and medications free?
The physician's evaluation and coordination may be included, but outside laboratory charges and prescription medication costs are generally separate unless specifically included in the employer agreement.
Can my spouse or children use the membership?
Dependent access depends on the coverage option selected by your employer and whether your spouse or children have been enrolled.
