Enrollment & Membership Terms
Before completing recurring-payment checkout, you will review and accept the current Urgent Care Membership Agreement.
Your enrollment will include agreement to:
- Recurring monthly billing for the membership plan you select.
- The definition and limitations of eligible online urgent-care visits.
- Clinical escalation when online treatment is not medically appropriate.
- Membership eligibility, household enrollment, renewal, and cancellation rules.
- Emergency-care exclusions and the requirement to seek emergency care when appropriate.
- Telehealth consent and electronic communications related to your care.
What Happens After You Join?

Frequently Asked Questions
Is this the same as health insurance?
No. The urgent-care membership is a direct-pay access membership for covered online urgent-care services. It is not health insurance and does not replace insurance or other coverage for emergency, hospital, specialist, testing, or other healthcare expenses.
Do I pay each time I submit an eligible visit?
No. Covered medically appropriate online urgent-care visits are included in the monthly membership while the membership is active.
Can I use the membership for any medical problem?
No. The service is intended for eligible non-emergency urgent-care concerns. Some symptoms and conditions require another type of evaluation.
Does membership guarantee a prescription?
No. Prescriptions are issued only when the reviewing physician determines medication is medically appropriate.
Who can be included in the family membership?
The family membership is intended for eligible enrolled members of the same household. Each person receiving care must be individually identified and enrolled before using the membership.
Where can I use the membership?
Online care is available only when the patient is physically located in a state where 24Sculpt Health has an appropriately licensed clinician and where the requested form of telehealth care is permitted.
Can I cancel?
Cancellation, renewal, effective-date, and billing terms are governed by the Urgent Care Membership Agreement presented during enrollment.
