Need employee exams in addition to DPC?
Employers do not need a DPC membership to use 24Sculpt Health for occupational examinations. Standard clinic-based exams may be scheduled directly, while recurring, on-site, disability and contract services are coordinated based on scope.
Use occupational services as neededβor combine them with an employer DPC program.
This gives employers one physician practice for employee primary care benefits and common work-related medical evaluations without requiring every service to be part of the DPC membership.

What should the employer provide?
For fitness-for-duty or return-to-work evaluations, provide the reason for evaluation, essential job duties, required employer forms and any specific clearance question you need addressed.
Who receives the result?
Work-status or certification information is handled according to the type of examination, applicable authorization and privacy requirements. The employer portal does not provide routine access to an employee's primary care record.
When is custom pricing used?
Custom pricing may apply to significant records review, extensive forms or reports, additional testing, recurring programs, travel, on-site work or other contract-specific requirements.

Does an employer need a DPC membership to schedule occupational exams?
No. DOT, fitness-for-duty and return-to-work services may be used separately from an employer DPC membership.
Can an employer schedule and pay for an employee's DOT or work-clearance exam?
Yes. The employer scheduling pathway is designed to allow the organization to initiate the appointment and identify who will be responsible for payment.
Do you offer on-site or regional occupational-health services?
South Louisiana travel, recurring programs and on-site services may be available for qualifying partnerships. Availability and pricing depend on location, volume, scheduling and service requirements.
Is Direct Primary Care health insurance?
No. Direct Primary Care is a membership-based primary care model. Employees should maintain appropriate insurance or other coverage for emergencies, hospitalization, surgery, specialists and services outside the membership.
Can this membership be paired with an employer health plan?
Yes. Employers may pair Direct Primary Care with a broader health plan or benefits strategy. Employers should consult their licensed benefits professionals regarding plan design and regulatory requirements.
How is employer pricing determined?
Employers with 5β19 enrolled employees pay $79 monthly for employee-only coverage, $149 for employee + spouse and $199 for employee + family. Employers with 20β49 enrolled employees pay $69, $139 and $189 monthly, respectively. Groups with 50 or more enrolled employees receive custom pricing.
Can employees add spouses or dependents?
Spouse and family options may be available depending on the employer's selected membership structure and eligibility rules.
Are there copays for included primary care visits?
Included primary care services do not require a separate visit copay. Services, testing, medications or treatment outside the membership may involve additional charges.
Can benefits brokers work with 24Sculpt Health?
Yes. 24Sculpt Health welcomes collaboration with licensed benefits brokers, third-party administrators and employer plan partners.
