Urgent Care Membership Agreement

Please review these terms before enrolling in an Individual or Family 24Sculpt Health Urgent Care Membership. This agreement governs recurring membership billing and access to eligible online urgent-care services.

Individual Membership $49.99 per month for one enrolled patient.
Family Membership $59.99 per month for eligible enrolled household members.
Recurring Billing Membership renews monthly until canceled according to this agreement.

Membership Terms

By enrolling, the member or responsible party agrees to the following terms.

1. Parties and Purpose

This Urgent Care Membership Agreement (“Agreement”) is between the enrolled patient or responsible party (“Member”) and 24Sculpt Health (“Practice”). The Agreement provides access to certain covered online urgent-care services in exchange for a recurring monthly membership fee.

This membership is a direct-pay access arrangement for defined online urgent-care services. It is not health insurance and does not replace comprehensive medical insurance or other coverage.

2. Membership Plans and Fees

Each patient who receives care must be individually identified and enrolled. The Practice may require information reasonably necessary to confirm household eligibility for a Family Membership.

3. Recurring Payment Authorization

By enrolling, the Member authorizes the Practice and its payment processor to charge the selected membership fee to the payment method provided at enrollment on a recurring monthly basis until the membership is canceled.

The monthly charge, billing frequency, and recurring nature of the membership will be disclosed before payment information is submitted. The Member is responsible for maintaining a valid payment method.

If a recurring payment fails, membership access may be suspended until the outstanding balance is paid or a valid payment method is provided.

4. Cancellation

The Member may cancel the recurring membership through the cancellation method provided by the Practice or payment platform. Cancellation should be made before the next scheduled billing date to prevent the next recurring charge.

Cancellation stops future recurring charges. Unless otherwise required by applicable law or stated at checkout, cancellation does not automatically refund a membership fee that has already been charged for the current billing period.

Membership access may continue through the end of the already-paid billing period unless the Practice provides otherwise or applicable law requires a different result.

5. Covered Membership Services

While the membership is active, enrolled patients may submit medically appropriate eligible online urgent-care visits without an additional per-visit membership charge.

Covered membership services may include:

  • Evaluation of eligible non-emergency urgent-care concerns through the Practice's online intake system.
  • Review of submitted symptoms, history, medications, allergies, photographs, and other requested information.
  • Clinical assessment by an appropriately licensed physician.
  • Electronic prescriptions when the reviewing physician determines medication is medically appropriate.
  • Care instructions and recommendations related to the submitted concern.

6. Meaning of “Unlimited”

“Unlimited” means there is no preset numerical limit on medically appropriate covered online urgent-care visits while the membership is active.

Unlimited access does not mean that every medical concern can or will be evaluated, diagnosed, treated, or prescribed for through asynchronous online care. Clinical appropriateness is determined by the reviewing physician.

The Practice may redirect a patient to telephone, video, laboratory, imaging, office-based, urgent-care, emergency, specialist, or other evaluation when medically appropriate.

7. Services Not Included

The membership does not include charges for:

  • Emergency department or hospital care.
  • Ambulance or emergency transportation.
  • Specialist services.
  • Laboratory testing, imaging, pathology, or outside facility fees.
  • Prescription medications or pharmacy charges.
  • Medical devices, supplies, procedures, or services provided by third parties.
  • Comprehensive primary-care management, annual wellness care, or routine chronic-disease management unless separately offered by the Practice.
  • Care that requires an in-person examination or testing.
  • Controlled-substance prescribing through the online urgent-care membership service.

8. No Guarantee of Treatment or Prescription

Submission of an online visit does not guarantee that the concern can be safely treated online and does not guarantee that a prescription, antibiotic, work note, school note, refill, or any particular treatment will be provided.

Medical decisions are based on the patient's submitted information, applicable standard of care, professional judgment, and legal requirements.

9. Patient Location and Service Availability

The patient must accurately report the state in which the patient is physically located at the time care is provided.

Services are available only when the Practice has an appropriately licensed clinician and when the requested method of telehealth care is permitted by applicable law and clinically appropriate.

A membership does not guarantee that online urgent-care services will be available in every state, for every condition, or through every telehealth modality.

10. Telehealth Consent

By requesting care through the membership, the patient consents to the use of telehealth and electronic communications for evaluation and treatment when permitted by law.

Telehealth has limitations, including the inability to perform a complete physical examination and the possibility that additional information, testing, video, telephone, or in-person evaluation may be required.

11. Emergency Care

Do not use this service for a medical emergency. Call 911 or go to the nearest emergency department for severe chest pain, severe difficulty breathing, stroke symptoms, severe bleeding, severe allergic reaction, loss of consciousness, suicidal or homicidal thoughts, confusion, or another emergency condition.

The membership does not provide emergency monitoring or continuous emergency coverage.

12. Member Responsibilities

The patient or responsible party agrees to:

  • Provide accurate and complete identity, contact, health, medication, allergy, pregnancy, pharmacy, and location information.
  • Provide requested photographs or other information when necessary for clinical review.
  • Update information when it changes.
  • Follow instructions to obtain higher-level or in-person care when recommended.
  • Not allow another person to use an individual membership or impersonate an enrolled patient.
  • Use the service only for legitimate healthcare needs.

13. Family Membership

A Family Membership covers only eligible household members who have been properly enrolled with the Practice. Each individual receiving medical care is treated as a separate patient and must have appropriate identifying and medical information on file.

A parent, legal guardian, or other legally authorized representative must provide consent for care of a minor or other patient who cannot legally consent for themselves, as required by applicable law.

14. Privacy and Electronic Communications

Medical information should be submitted only through communication methods designated by the Practice for clinical information.

The Practice will handle protected health information in accordance with its Notice of Privacy Practices and applicable privacy law.

The Member authorizes routine electronic communications related to membership, billing, enrollment, visit status, and care coordination using the contact information provided.

15. Membership Suspension or Termination by the Practice

The Practice may suspend or terminate membership for nonpayment, fraud, misuse of the service, threats or abusive conduct, repeated attempts to use the service for inappropriate or prohibited purposes, or other material breach of this Agreement, subject to applicable law and professional obligations.

Termination of a membership does not prevent the patient from seeking medical care elsewhere.

16. Changes to Membership Terms or Fees

The Practice may modify membership pricing, covered services, eligibility, or terms prospectively. When required, notice will be provided before a change becomes effective. Continued enrollment after the effective date of a properly disclosed change constitutes acceptance to the extent permitted by law.

17. No Insurance Billing for Membership Fee

The membership fee is a direct-pay charge for membership access and is not represented as an insurance premium. The Practice does not promise that the membership fee is reimbursable by any health plan, HSA, FSA, employer, or other payer.

18. Governing Law and State-Specific Requirements

Medical services are subject to the laws and professional standards applicable in the state where the patient is physically located when care is provided. This Agreement will be interpreted consistently with applicable federal and state law. If a provision conflicts with a mandatory legal requirement, the mandatory requirement controls.

19. Entire Agreement

This Agreement, together with the applicable checkout disclosures, telehealth consent, Notice of Privacy Practices, and other incorporated Practice policies, constitutes the terms governing the Urgent Care Membership. If there is a conflict, the most specific applicable patient-facing term controls to the extent permitted by law.
